Case Studies

Case Studies

We're working with community members who have agreed to participate in the case studies by monitoring their progress from the start of the process to the very end. Case studies involve an in-depth intake process, check-ins, identifying their needs with a community navigator, participating in social prescriptions and follow-ups.

New Health & Care Model

New Health & Care Model

We're convinced this new health and care model will revolutionize the way we approach health, care and treatment as we're taking into account the whole person and what truly matters to them. Our collaboration with social workers and health professionals allows us to map out the most effective and customized person-centric approach.
